Host Home

Host Home services provide a stable, supportive living environment where individuals receive care in a private home setting. Care is delivered by a qualified Host Home Provider, with oversight and support from Tree of Life.

For individuals on the DD waiver, who require a structured residential environment, benefit from consistent routines and individualized support and prefer a family-style living arrangement rather than a larger setting.

 

Supports may include:

Assistance with daily living activities

Health and safety monitoring

Skill development and independence support

Transportation and community involvement

Coordination with case managers and service teams

Family Caregiver

Family Caregiver services allow a qualified family member to serve as the paid caregiver while the individual continues to live with their family. Care is delivered by a family member, with oversight and support from Tree of Life.

For individuals on the DD waiver, who require a structured residential environment, benefit from consistent routines and individualized support and prefer a family-style living arrangement rather than a larger setting.

 

Supports may include:

Assistance with daily living activities

Health and safety monitoring

Skill development and independence support

Transportation and community involvement

Coordination with case managers and service teams

Respite

Respite services provide short-term support to ensure continuity of care when a primary caregiver is unavailable. Services are delivered according to authorized hours and individual needs.

This service may be appropriate for individuals who are on the SLS and CES waivers, require temporary coverage, live with family or a Host Home Provider and need consistent support during caregiver absences.

 

Supports may include:

Short-term supervision and care

Assistance with daily routines

Health and safety monitoring

Supported Community Connections

Supported Community Connections focus on helping individuals build skills, relationships, and independence through meaningful community engagement.

This service may be appropriate for individuals who are on the DD, SLS, and CES waivers, want to increase community involvement, are developing social, vocational, or daily living skills, and benefit from structured community-based support

 

Supports may include:

Skill-building activities

Social and recreational participation

Community navigation and transportation support

Goal-oriented engagement aligned with the service plan

Agency Oversight

 

Tree of Life Supports & Services emphasizes consistency, safety, and accountability across all services. This includes:

Ongoing monitoring and provider oversight

Regular communication with individuals and case managers

Documentation and compliance with Colorado Medicaid requirements

Clear expectations for providers and support staff
